I understand this form must be completed and submitted online in order to participate in CAPS programs and trainings. I also understand that Crisis Assessment & Prevention Systems will provide the training and support necessary to insure that all participants obtain the skills necessary to prevent and manage aggressive behavior. Some workshops require a certain physical skill level and knowledge competency in order to obtain certification. I understand that this training requires physical skills that require practice and supervision from a trained professional. I further understand that Crisis Assessment and Prevention Systems will only be responsible for injuries that may occur from negligence due to direct physical contact from CAPS trainers. I assume all responsibilities for my health and safety during CAPS facilitated programs.

*CAPS must receive fees within 5 business days of the registration date in order to guarantee a space in the workshop.  Fees listed are individual rates.  Payment terms and fees may vary for groups.
